Outcome 4: Produce documentation to support the delivery of physical therapy services.

The artifact that I used to demonstrate this outcome were SOAP notes written from Keep Stepping. I used two from the first round of Keep Stepping and two from the second round of Keep Stepping. I included a SOAP note from the first day of Keep Stepping and the last day of Keep Stepping to show progress in writing SOAP notes. I included from both rounds of Keep Stepping to show the different formats we did. In the first round of Keep Stepping we made our own format and had to type everything out ourselves. This was a good way to start because it gave us better practice on what goes in each section and what needs to be written in each. Then during the second round of Keep Stepping we used an electronic based format called WebPT. This gave us a feel for online documenting and how it looks a little different than the first time. This format gave up help on what needs to be included on each section and that was even broken down. It gave a good feel on what our clinicals might use for documenting.
